What to Expect When You Refer a Patient

We aim to make the referral process straightforward, collaborative, and respectful of your existing relationship with the patient.

  • Initial Fit & Intake:  Patients begin with a brief introductory call or comprehensive consultation, depending on their needs. We assess fit, goals, and whether a functional medicine approach is appropriate.

  • Comprehensive Evaluation:  When indicated, care includes a detailed review of medical history, symptoms, lifestyle factors, and relevant laboratory data to identify underlying contributors to the patient’s concerns.

  • Collaborative Care Planning:  Recommendations are developed with consideration for the patient’s current care plan. When appropriate, we coordinate with referring providers to support alignment and continuity.

  • Communication & Follow-Up:  We are available to communicate with referring providers regarding findings, recommendations, and care direction. Patients are encouraged to maintain their relationships with their primary and specialty providers.

  • Scope & Role:  Adaptive Wellness typically serves in a consultative or adjunctive role, supporting patients with additional insight and strategy while preserving the central role of their existing care team.

  • When to Consider Referral:  Referral may be helpful when patients have persistent or complex symptoms, are not progressing as expected, or would benefit from a deeper evaluation of underlying contributors.
